Consumers Union developed a new consumer brochure that accurately conveys information about the new health premium tax credits and gets consumers to take action. The Tax Credit Brochure was created as a fillable PDF (in order to provide information on local sources of help) but this SAMPLE version shows what the brochure looks like once completed.
All Brochure versions include information on how to contact the Health Plan Marketplace in the state. This version also allows a local assistor (like a navigator, broker, tax preparer or other) to include their contact information. An alternate version does not require this information before completing.
NOTE: Tax Credit Brochure information is specific to the calendar year of coverage. You can use this form to be notified of yearly updates to the brochure.
